Admission Criteria
A candidate must have been hospitalized for an acute medical contition for 3 midnights or been hospitalized in the last 30 days with a notable decline in medical and functional abilities related to orthopedic condition prior to consideration for program eligibility.
A candidate must possess both functional impairments & medical conditions requiring skilled nursing and rehabilitation services

Must possess approved Medicare - Part A funding for inpatient rehabilitation services or other approved funding source.
Must be able to actively participate in up to 2 hours of rehabilitation therapy per day
